01 "An Act relating to state budget information that must be included on permanent fund                                    
02 dividend application forms; and providing for an effective date."                                                       
03 BE IT ENACTED BY THE LEGISLATURE OF THE STATE OF ALASKA:                                                                
04    * Section 1.  AS 43.23.015(b) is amended to read:                                                                  
05            (b)  The department shall prescribe and furnish an application form for                                      
06       claiming a permanent fund dividend. The application must include notice of the                                    
07       penalties provided for under AS 43.23.035 and contain a statement of eligibility and a                            
08       certification of residency.  The application must also include summary information                            
09       about the state budget, including at least the following:                                                     
10                 (1)  a pie chart depicting sources of income and estimated total                                    
11       amounts of income from each source expected to be received by the state during                                
12       the current fiscal year;                                                                                      
13                 (2)  a pie chart depicting expenditures expected to be made by the                                  
14       state during the current fiscal year;                                                                         
01                 (3)  a graph depicting the value of the budget reserve fund                                         
02       established under art. IX, sec. 17, Constitution of the State of Alaska, at the end                           
03       of each of the prior five fiscal years, and the projected value of the budget reserve                         
04       fund at the end of the current fiscal year; and                                                               
05                 (4)  a graph depicting the amount of money expected to be added to                                  
06       the budget reserve fund during the current fiscal year and the amount expected                                
07       to be withdrawn from the budget reserve fund during the current fiscal year.                                  
08    * Sec. 2.  The uncodified law of the State of Alaska is amended by adding a new section to                         
09 read:                                                                                                                   
10       APPLICABILITY.  AS 43.23.015(b), as amended by sec. 1 of this Act, applies                                        
11 beginning with the permanent fund dividend application for 2005 and applies to applications                             
12 for years thereafter.                                                                                                   
13    * Sec. 3.  This Act takes effect July 1, 2005.
