00 HOUSE BILL NO. 318 01 "An Act relating to a health insurance uniform prescription drug information card; and 02 providing for an effective date." 03 BE IT ENACTED BY THE LEGISLATURE OF THE STATE OF ALASKA: 04  * Section 1. AS 21.54 is amended by adding a new section to article 2 to read: 05 Sec. 21.54.180. Uniform prescription drug information card. (a) A health 06 care insurer that issues or reissues a card for prescription drug claims processing shall 07 include uniform prescription drug information on the card as required in this section. 08 The director shall by regulation specify the information to be contained on a uniform 09 prescription drug information card and shall require, at a minimum, that a uniform 10 prescription drug information card contain 11 (1) the health care insurer's business identification number; 12 (2) the covered individual's identification number; 13 (3) the telephone number of the pharmacy benefits manager, if 14 different from the health care insurer; 01 (4) the processor control number, if required for adjudication; 02 (5) the group number, if required for adjudication; 03 (6) the person code, if required for adjudication. 04 (b) A uniform prescription drug information card must 05 (1) contain in a clear, readable, and understandable manner all 06 information necessary for prescription drug claims submission; and 07 (2) be formatted and arranged in a manner required by the health care 08 insurer for processing claims. 09 (c) This section does not apply to a health care insurer providing prescription 10 drug coverage under an excepted benefits policy. 11 (d) In this section, 12 (1) "device" has the meaning given in AS 08.80.480; 13 (2) "excepted benefits" has the meaning given in AS 21.54.160; 14 (3) "prescription drug" has the meaning given in AS 08.80.480 and 15 includes a device. 16  * Sec. 2. This Act takes effect July 1, 2003.
