Page 2, line 30, following "dependents.":                                                                                       
    Insert "However, the insurer shall make coverage of prescription                                                            
contraceptives and voluntary sterilization procedures contingent on                                                             
notification of the primary insured before an enrolled spouse or                                                                
dependent is administered or dispensed prescription contraceptives or                                                           
undergoes a voluntary sterilization procedure."
