00 HOUSE BILL NO. 468 01 "An Act relating to disclosure of employment information on a medical assistance 02 application and a hospital intake report; and requiring the Department of Health and 03 Social Services to prepare and publicize a report pertaining to employers who do not 04 provide health insurance." 05 BE IT ENACTED BY THE LEGISLATURE OF THE STATE OF ALASKA: 06 * Section 1. AS 18.23 is amended by adding a new section to article 3 to read: 07 Sec. 18.23.110. Hospital intake form and report. (a) A hospital shall include 08 on an intake form of a patient who is seeking medical treatment the name and address 09 of the employer of the patient or, if the patient is a dependent of another person, of the 10 person financially responsible for the patient. 11 (b) A hospital shall provide to the Department of Health and Social Services, 12 before July 1 of each year, the name and address of each employer identified on a 13 hospital intake form under (a) of this section of a patient who is not covered by 14 insurance.

01 * Sec. 2. AS 47.07 is amended by adding new sections to read: 02 Sec. 47.07.061. Medical assistance application. The Department of Health 03 and Social Services shall include on the application for medical assistance the name 04 and address of the employer of the person applying for medical assistance. If the 05 applicant for or recipient of medical assistance is a dependent of another person, the 06 application must include the name and address of the employer of the person 07 financially responsible for the applicant or recipient. 08 Sec. 47.07.062. Employment report on medical assistance recipients and 09 applicants and on uninsured hospital patients. (a) The Department of Health and 10 Social Services shall prepare and transmit, not later that January 20 of each year, a 11 report to the legislature of the names and addresses of the employers listed on the 12 medical assistance application under AS 47.07.061 and the hospital intake form and 13 report under AS 18.23.110(b), along with the number of applicants for and recipients 14 of medical assistance listing that employer on an application for medical assistance, 15 and the number of uninsured hospital patients listing that employer on a hospital 16 intake form. 17 (b) The report prepared under (a) of this section shall be made available to the 18 public. 19 (c) In this section, "employer" includes a business and its subsidiaries located 20 in the state that employs more than 25 persons either as employees or contractors.
